What is melasma, which is different from blemishes?
Melasma is not simply a problem of excessive melanin production causing pigmentation on the skin.
In the deep layers of the skin, pigment cells called melanocytes become overactive,
and the melanin produced by these cells spreads widely and repeatedly,
penetrating even into the dermis, resulting in a complex pigment disorder that persists long-term.
External stimuli, especially UV radiation or hormonal changes, stimulate melanocytes,
and in this process, subtle inflammatory responses and vascular factors within the skin act together,
characterized by pigment spreading in a blurry, widespread pattern rather than simple spot-like forms.
Histologically, the number of melanocytes is within the normal range,
but the size of the cells and their dendritic processes significantly increase,
causing melanin to be transferred more abundantly and further to surrounding cells.
Additionally, in some patients, subtle capillary dilation or flushing reactions are observed in the melasma area,
and in such cases, it is sometimes described as a separate category called vascular melasma.
Melasma is less a disease in itself and more a result of the skin reacting easily
and originating from various stimuli when in a structurally unstable state.
Therefore, treatment that corrects its root causes and structure, rather than just simple pigment removal, is important.
While general pigment disorders also require dual toning, for melasma in particular, dual toning is essential, not just general toning.
