Differentiation 5: Only Lentigo? Design-Based Treatment Considering All Pigmentation

Most clients who visit us for lentigo removal also have dermal pigmentation such as melasma, in addition to lentigo.

While it's good to target and remove only lentigo, we believe clients ultimately wish to remove all uneven pigmentation.

After thorough consultation with the client and using diagnostic equipment, we classify the pigmentation on the face and then determine the treatment direction: whether to target only epidermal pigmentation or to remove all overall pigmentation.

Why do lentigo spots appear?

Solar lentigo is a flat, brown pigmented lesion primarily caused by UV exposure and skin aging,

where melanocytes become overactive, leading to excessive pigment deposition in specific areas.

Repeated exposure to UV radiation continuously stimulates melanin production,

and as one ages, the skin's regenerative capacity and pigment metabolism decrease, causing them to gradually become darker and spread wider.

They primarily appear on the face, back of hands, and arms, characterized by distinct borders and a dark brown color.

 

Location of Lentigo Distribution

Lentigo (solar lentigo) primarily occurs confined to the epidermal layer (Epidermis) of the skin,

specifically, melanocytes located in the basal layer become overactive due to UV stimulation,

resulting in a localized accumulation of melanin pigment.

Histopathologically, they do not penetrate the dermis, and represent a state of excessive melanin deposition within the epidermis,

with relatively clear borders observed between them and the surrounding normal tissue.

For this reason, they typically respond well to epidermal-targeted laser treatments such as Q-switched 532nm,

It is distinguished from melasma or mixed pigment lesions that affect the dermis.

 

532nm Wavelength

The 532nm wavelength is a green wavelength that falls within the range with the highest absorption rate for melanin pigment, selectively acting on pigment lesions confined to the epidermis of the skin.

The 532nm laser is typically used in a Q-switched manner, which instantaneously irradiates high-power energy in ultra-short pulses of several nanoseconds, destroying only the pigment particles.

These short pulses are selectively absorbed by melanin and converted into thermal energy, simultaneously generating a photomechanical effect, which breaks down melanin particles.

Some of the broken-down pigment flakes off as scabs, while others are expelled through the lymphatic system by macrophages.

As these destroyed pigments are removed, the lentigo visually lightens or disappears.
