Inside the raised, convex scar, microscopic channels are created at 1-3mm intervals using a CO₂ or Er:YAG laser. The purpose of this is to break the connections of excessively proliferated collagen tissue.

This procedure softens the scar tissue, gradually flattening the scar.

This procedure penetrates deeper into the dermal layer than conventional superficial ablative fractional lasers. Therefore, it can be considered for papular or hypertrophic scars that are difficult to improve with simple surface ablation.
